I'm Tracy Iona, a brain aneurysm and stroke survivor and the founder of ClearPath Recovery Solutions. I created ClearPath because the gap between discharge and real life is where people get lost — and where families need practical, compassionate guidance.
The moment I realized the system wasn't built for real life
After my brain aneurysm and stroke, I learned that leaving the hospital isn't the finish line — it's the start of a new job: managing appointments, medications, symptoms, paperwork, fatigue, emotions, and daily life.
What surprised me most wasn't the medical care — it was how quickly support drops off once you're home. Families are expected to coordinate everything while they're exhausted, scared, and overwhelmed.
That's why I built ClearPath: a non-clinical, virtual navigation and coaching service that helps survivors and caregivers create a clear plan, use simple tools, and feel supported during the transition home.
