Is this medical advice or therapy?
No. ClearPath Recovery Solutions provides non-clinical recovery navigation and coaching. We do not provide medical advice, diagnosis, therapy, or emergency services.
Do you take insurance?
Not at this time. If you have questions about payment, email [email protected].
Do you work with caregivers, or only survivors?
Both! ClearPath supports survivors, caregivers, or the whole family — whatever combination makes sense for your situation. You don’t have to choose one or the other.
Is this only for Long Island?
ClearPath is based on Long Island, NY, but all services are delivered virtually — across New York State and beyond.
How fast can we start?
We respond within 1 business day and aim to start within 48–72 hours post-discharge.
What’s included in the toolkit?
The Home Transition Navigator™ Toolkit includes checklists, trackers, planners, and worksheets — all designed with large fonts, high contrast, and simple layouts for easy use.
What if I’m not sure which package is right for me?
Start with the free 45-minute Transition Call. We’ll figure out what’s happening, what you need most, and which level of support fits.
Can I get support for someone else?
Yes. Family members and loved ones often reach out on behalf of a survivor. We’ll work with whoever is involved in the recovery process.
Is there a contract or long-term commitment?
No long-term contracts. The Home Transition Navigation package runs on a month-to-month basis within a 90-day window.
Do you offer group support?
A small weekly support group (via Zoom, fewer than 6 members) is coming soon. Contact us to express interest.
